Delay repay - oyster card

Does anyone know how to claim compensation for a ticket paid for on an oyster card due to a delayed train?

The delay repay claim form from Greater Anglia has this option on their claim form. You have to supply a printed statement showing the date of travel in question along with the form. I presumed it would be the same for all the rail companies,but maybe not.

First Capital Connect says you can do it but doesn't explain how.

Next thing we need is to have the option to have our online accounts credited rather than have old-fashioned bits of of paper posted to us.
